The Community Action Program Specialist provides a vital link between clients in need of services and the resources that can address their needs. The CAP Specialist determines eligibility for services that help prevent homelessness and promote self-sustainability through the provision of food, utility assistance, crisis case management, and community referrals. The ideal candidate will possess a passion for serving underprivileged individuals and households, keen attention to detail, and a desire to work within a high-performing team.

ESSENTIAL FUNCTIONS

Duties include, but are not limited to, the following:

  • Conducts outreach, intake, assessment, eligibility determination, and referral for Guadalupe clients. Demonstrates professionalism, compassion, and sensitivity in all interactions with clients.
  • Identifies and connects clients with appropriate services within the agency and through other providers to address individual and household needs. Maintains extensive knowledge of community resources. Advocates on behalf of clients as needed.
  • Keeps accurate and up-to-date case files, collects all information needed for reporting purposes, and assists with file audits.
  • Monitors and maintains cltent electronic (e) caseload files and maintains confidentiality.
  • Performs related administrative functions and other similar duties as necessary.
  • Provides information regarding resources and makes referrals for services. Assists clients in receiving services, completing forms and applications, interprets and explains communication (email/phone), schedules appointments for intake/food services, and provides or arranges for transportation.
  • Assist with weekly food distribution, food rescue, and home deliveries.
  • Conducts outreach in the community (service target area) to identify and recruit people in need of services offered by the department.
  • Notifies the community about services via social media, Town website, flyers, and marketing materials.
  • Inputs data into Dynamics database for Maricopa County case documentation.
  • Prepares various program reports as required.
  • Coordinates activities and events that promote Town/CAP services and programs.
  • Prepares funding proposals as needed.
  • Assists with maintaining CAP forms, materials, and supplies related to warehouse.
  • Collaborates with supervisor and town staff and takes a proactive role in suggesting improvements for services and operations of the department.
  • Performs other duties that may be assigned.

Qualifications

  • Bachelor's degree in public administration, political science, social work desired or associate degree in a related field. Other combinations of experience in human services and social welfare programs and education in information systems highly desired.
  • May require use of Town vehicles on Town business. Individuals must be physically capable of operating vehicles safely, possess a valid Arizona driver's license and a good driving record.
  • At least two years of case management, eligibility determination, or social work experience.
  • Responsible work history indicating dependability, initiative, flexibility, and ability to follow directions.
  • Sensitivity to the needs of populations from backgrounds of disadvantage.
  • Knowledge of principles, practices, and methods of community social services, human services, or public health.
  • General knowledge of government assistance programs and services, such as food distribution/security/hunger, and other county resources.
  • Budgetary and financial knowledge are beneficial, as well as basic math for calculations.
  • Electronic record keeping from first client interaction to completion as well as reporting.
  • Case methods including legal terminology for electronic notes, techniques for follow-up, and the principles and practices of social work and supervision of volunteers as needed.
  • Ability to communicate effectively in English and Spanish, both verbally and in writing, and translate as needed.
  • Ability to lift and move boxes of food and supplies (at least 25 lbs.) and perform repetitive physical tasks.
