Case Manager: JFCS is currently hiring Case Managers for the Children's and High Needs behavioral health teams.  Competitive pay, great benefits, and other financial incentives are attached to all case-management positions at JFCS.  We currently have openings at each of our four Valley clinics, located in Phoenix, Gilbert, Glendale, and the West Valley. 

Duties include:

  • Provide Case Management services to assigned clients, following appropriate timelines for monthly contact, quarterly home/community visits, attending medication checks, etc. 
  • Assist clients in finding services as clinically indicated, and work to decrease behaviors that will allow the client to be stable in the community and home.
  • Completes referrals, prior authorizations, and other paperwork required by the health plans to procure client the services that are clinically indicated.
  • Responsible for coordinating and facilitating Child and Family Team (CFT) Meetings.
  • Collaborates with the clinical team to ensure that Integrated Service Plans, Child and Family Team (CFT) Meetings, and all required paperwork is completed accurately and in a timely manner. 
  • Collaborates and coordinates services with other involved agencies, such as the Department of Child Safety, Probation/Parole Departments, Division of Developmental Disabilities, schools, employers, and other providers.
  • Will inquire about the client's health & wellness needs and assist in coordinating primary care or other supportive services with internal staff or external parties to ensure the client's whole health is addressed.  

Qualifications

Must meet one of the following education/experience requirements:

  • High School Diploma/GED required and 4 years of Behavioral Health experience
  • A.A. Degree and two years of Behavioral Health experience
  • Bachelor's Degree in Social Work, Counseling, Marriage and Family Therapy, Psychology or related field.

  • Level 1 Fingerprint Clearance Card without Restriction(s)
  • Meet Agency requirements for MVR (Motor Vehicle Record) report
